RN Care Manager Weekend

Role Overview: Provides care coordination and discharge planning from admission to discharge, ensuring safe patient transitions and promoting quality, safety, and efficient healthcare delivery. Key Responsibilities: Assesses patient needs, develops individualized care plans, advocates for patients, and facilitates communication among healthcare teams. Addresses complex psychosocial needs and promotes health education to improve outcomes. Qualifications & Skills: Requires a Bachelor’s Degree in Nursing, current RN license, and at least 1 year of acute care hospital experience. BLS certification is mandatory; case management certification and additional experience in care or utilization management are preferred. Clinical expertise, critical thinking, compassion, and strong communication are essential.
